Hook: Small hook, no, make that tiny hook. Here a Dai-ichi 1401, size 24 (according to the box -- it's actually more like a 28)
Thread: UNI, size 8/0
Body: Anything that will give a two-tone effect, such as heavy white thread ribbed over a black tying thread layer, or two moose mane hairs, one black and one white
Wing: Single reversed hackle wing ('wonderwing'), tied downwing, or diptera style
Thorax: Shaggy, but not too coarse. E.g. hare's ear dubbing
Hackle: Couple of turns of a grizzly hackle feather, correct size, wrapped back to front over the thorax

Basically, the pattern description speaks for itself. I'm not sure fish will actually fall for this, but to my human eye, this pattern is remarkably true to a real chironomid or culicid adult gnat. I'd assume a fish sees nothing more than a tiny dimple in the water's surface, with an even tinier speck in the middle of it...
